Note: This is only a quote!  The Agreement does not take effect until it is signed
and returned to our office.  We will call to confirm receipt of the Agreement contract.

Please allow Main Line Executive access to the location at least forty-five (45) minutes
prior to start time.  Two (2) rectangular tables (preferred) and access to one grounded
electrical outlet are the responsibility of the CLIENT.  Main Line Executive does not
provide tablecloths or skirting, please make appropriate arrangements with the caterer
or banquet facility. 

Check must be made payable to MAIN LINE EXECUTIVE.  In the event of breach of contract,
damages shall not exceed the total fee as indicated above.  Any changes to this agreement
must be submitted in writing.  If Client must cancel, notice must be received at least
forty-eight (48) hours prior to the event or the full balance will be due.

Although not required, any gratuity paid to the entertainer becomes his sole property
and should be paid directly to the DJ.
